B2 English Requirement for Care Workers: Complete Guide 2026

What Changed and Why

From January 2026, the minimum English language level for the Health and Care Worker visa (for care worker and senior care worker roles) increased from CEFR B1 (intermediate) to CEFR B2 (upper intermediate).

The government's rationale for the change includes:

  • Patient safety: Care workers need to communicate clearly with vulnerable people, understand medication instructions, and write accurate care notes
  • Safeguarding: Higher English proficiency helps workers identify and report safeguarding concerns
  • Integration: B2 level enables workers to participate more fully in workplace and community life
  • Professional standards: Aligns care worker English requirements more closely with other health professions

Important: IELTS for UKVI Only

You must take the IELTS for UKVI version at an approved Secure English Language Test (SELT) centre. Regular IELTS taken at universities or non-SELT centres is not accepted for visa purposes. The test content is the same, but you must book specifically the UKVI version. Both Academic and General Training are accepted.

Cost: Approximately GBP 200-210 per test. Test validity: IELTS for UKVI results are valid for 2 years from the test date. Book at ielts.org and select "IELTS for UKVI".

OET as an Alternative

The Occupational English Test (OET) is an alternative to IELTS that is specifically designed for healthcare professionals. Many care workers find OET more relevant because the test content is based on healthcare scenarios.

Who Is Exempt

You do not need to take an English language test if:

  • You are a national of a majority English-speaking country: This includes the USA, Canada, Australia, New Zealand, Jamaica, Barbados, Trinidad and Tobago, and others. Check the full UKVI list
  • You have a degree taught in English: Your degree must be from a UK institution or from an institution in a majority English-speaking country. You may need to provide a certificate from UK ENIC (formerly NARIC) confirming the degree is equivalent to a UK Bachelor's degree
  • You have previously passed an approved English test at B2 or above within the validity period (usually 2 years)

Impact on Visa Extensions

If you already hold a care worker visa and need to extend, the English language requirement at the time of your extension application applies. This means:

  • If you extend after January 2026, you may need to demonstrate B2 English
  • If you already passed B2 or above, your existing test result may still be valid (check the 2-year validity)
  • If you originally passed at B1 only, you may need to retake the test at B2 level
  • Check the specific UKVI guidance at the time of your extension, as transitional arrangements may apply

Start Preparing Now

Even if your visa does not expire for another year or two, start preparing for the B2 requirement now. The jump from B1 (IELTS 4.0) to B2 (IELTS 5.5) is significant and most people need several months of study and practice.

English for ILR (Still B1)

Good news: the English language requirement for Indefinite Leave to Remain (ILR) settlement remains at B1 level. For ILR, you can use the IELTS Life Skills test (B1 level), which only tests Listening and Speaking and costs approximately GBP 150.
